This Registration Form is to be used for registering for the 2024 Wilk-Amite Youth Football and Cheerleader program.   

Wilk-Amite Youth Football and Cheerleader participants must be between 5 and 12 years of age. Qualifying age is determined based upon the participant’s age on August 1st immediately preceding the current season.   

 
Wilk-Amite Youth Football participants must have a valid State Issued IDENTIFICATION CARD to be certified to participate in the Football program .   Wilk-Amite Youth Cheerleader Participants must have a valid Birth Certificate to be certified to participate in the Cheerleader  program. If your child intends to participate in Wilk-Amite Youth Football or Cheerleading, please obtain the participant’s State Issued IDENTIFICATION CARD or birth certificate as soon as you can. 
 
Participants in Wilk-Amite Youth Football and Cheerleader must maintain a minimum 2.0 GPA, or equivalent with no grade in any subject being an “F” or otherwise unsatisfactory (“U”) and no disciplinary or conduct issues in school, on the bus, at home or in the community to be eligible to participate in Wilk-Amite Youth Football and Cheerleader.   Exceptions to this requirement may be made by the Commissioner after consulting with the participant, participant’s parents and head coach with the most important factor to be considered is the participant’s and parent’s attitude toward improvement.   

Wilk-Amite Youth Football Coaches, Administrators, and Athletes will be provided Individual Membership in USA Football’s Affiliate Initiative.  In addition to educational services and resources designed to elevate the operational standards for youth football organizations, USA Football Affiliation provides additional benefits including but not limited to  year-round insurance coverage for football activities (flag and tackle).
 
INSURANCE COVERAGE THAT COMES WITH USA FOOTBALL MEMBERSHIP:
Accident Medical Expense: $100,000
Accidental Death and Dismemberment: $10,000
Emergency Room: 100% URC*
Dental Benefit Amount: 100% of URC* up to $250 per injury
Physical Therapy Benefit Amount: 100% of URC*, $100 max per visit, 10 visits maximum
Durable Medical Equipment: Included
Deductible: $250 per injury
*URC = Usual, Reasonable & Customary charges/fees
 
The Accident policy will pay medical bills for injured participants. The insurance coverage is excess, which means that other insurance, such as parent’s health insurance, must respond first before the excess Accident policy will pay any bills.
 
Registration fees for participants are used to provide a quality program for participants including USA Football Membership, website fees, supplies and equipment.  

Football Fees -$50.00
Cheerleader Fees - $50.00

 
Wilk-Amite Youth Action Network is forced to make decisions and pay insurance and other fees months before the season starts.   Therefore,  no refunds are given, except; registered participants may request a partial refund if the program is cancelled.

Wilk-Amite Youth Football and Cheerleader plays an important role in promoting the physical, social and emotional development of children. It is therefore essential for parents to encourage youth athletes to embrace the values of good sportsmanship. Moreover, adults involved in youth sports events should be models of good sportsmanship and should lead by example by demonstrating fairness, respect and self-control.

I therefore pledge to be responsible for my words and actions while attending or participating in a youth sports event and shall conform my behavior to the following code of conduct:

1. I will not engage in unsportsmanlike conduct with any coach, parent, player, participant, official or any other attendee.
2.  I will not encourage my child, or any other person, to engage in unsportsmanlike conduct with any coach, parent, player, participant, official or any other attendee.
3. I will not engage in any behavior which would endanger the health, safety or well-being of any coach, parent, player, participant, official or any other attendee.
4. I will not encourage my child, or any other person, to engage in any behavior which would endanger the health, safety or well-being of any coach, parent, player, participant, official or any other attendee.
5. I will not use drugs or alcohol while at a youth sports event and will not attend, coach, officiate or participate in a youth sports event while under the influence of drugs or alcohol.
6. I will not permit my child, or encourage any other person, to use drugs or alcohol at a youth sports event and will not permit my child, or encourage any other person, to attend, coach, officiate or participate in a youth sports event while under the influence of drugs or alcohol.
7. I will not engage in the use of profanity.
8.  I will not encourage my child, or any other person, to engage in the use of profanity.
9. I will treat any coach, parent, player, participant, official or any other attendee with respect regardless of race, creed, color, national origin, sex, sexual orientation or ability.
10.  I will encourage my child to treat any coach, parent, player, participant, official or any other attendee with respect regardless of race, creed, color, national origin, sex, sexual orientation or ability.
11.  I will not engage in verbal or physical threats or abuse aimed at any coach, parent, player, participant, official or any other attendee.
12.  I will not encourage my child, or any other person, to engage in verbal or physical threats or abuse aimed at any coach, parent, player, participant, official or any other attendee.
13.  I will not initiate a fight or scuffle with any coach, parent, player, participant, official or any other attendee.
14.  I will not encourage my child, or any other person, to initiate a fight or scuffle with any coach, parent, player, participant, official or any other attendee.

I hereby agree that if I fail to conform my conduct to the foregoing while attending a Wilk-Amite Youth Football and Cheerleader sports event I will be subject to actions taken by the Wilk-Amite Youth Action Network/Wilk-Amite Youth Football and Cheerleader, including but not limited to the following in any order or combination:

1.      Suspension or immediate ejection from a youth sports event.
2.      Suspension from multiple youth sports events.
3.      Season suspension or multiple season suspension.
4.      Removal of my child/children from Wilk-Amite Youth Football and Cheerleader program.
